I had a problem once, where I made a payment that didn't show up but had been billed to my credit card. To make a long story short, I was told they have a "front office system" and a "back office system" and they are not well integrated. The front office system did not show my payment but the back office system did and they had to manually update the front office system
If you just call on the phone, the run-of-the-mill reservation agent can only get into the front office system but not the back office system. If they have to do something unusual, they may have to go, or get someone with authority to go, into the back office system.
I'll bet this is in part the source of these problems. Maybe one address in the front office system, which is current, but if there's a problem or for some other reason, they go to the back office system to do some type of more thorough work - but wait - the info in the back office system is wrong, and they don't realize it, because they don't use the back office system in the usual reservation process. The systems aren't integrated to match updates in addresses, etc. This is my guess.
