My brother and I are contemplating going to visit our sister in New Orleans, but he won't know for awhile if he'll be able to get the time off. Can I book MY airfare NOW and add him as my companion on this trip at a later date, or do I have to book my ticket and his at the same time?

As long as there is a seat available on the plane when you are ready to book, you can add him at a later time. You can also add him now and cancel him later if things don't pan out.
 
Just to confirm- your brother is registered as your companion, correct?
You can change the person named as your companion up to 3 times per calendar year. If her brother is not named as her companion now, she can change it when he is certain that he can make the trip.
