The last full day of our September trip has Morning EMH for Animal Kingdom, and Evening EMH for Magic Kingdom. I've seriously debated talking my wife into doing a 20-ride, four-park challenge. Five rides at each of the four parks.
And because we never get a rental car, we'd be doing it with only Disney transportation.
I don't know if I'd set a firm 5 rides per park. I'm considering something similar in Early January but different numbers of rides at each park. Only a couple in AK and hopefully a whole bunch in MK. Obviously you would start at DAK. Since you're using Disney transportation for the day I'd look at hitting DHS next. I don't know what's on you're list, but if the current FP+ rules are still in force I'd consider using you're 3 FP+'s here for TSMM, TOT, & Tier 2 of your choosing.
From there walk or take the Boat to DHS, I'd hit the Kiosks at the International Gateway to see if you can get a 4th FP+ for Soarin' (first choice) or TT (Backup). I'd pick Soarin' first because you could single rider line TT.
From there take the monorail to MK. Upon arrival get FP+ for biggest headliner left realizing that lines get shorter as the EMH continue. If you can't get a FP for 7DMT I'd plan on getting in line right before closing if the line never get's short.
By Going AK->DHS->EPCOT->MK you minimize the amount of busses you need to take during the day.
