Please explain Rider Swap with Fast Pass

Planning to use rider swap this year for the first time ever...

When making Fast Passes, do I make reservations for ALL adults, and just take turns that way? OR... do I make FP reservations for one groups, a different reservation for another group, then group one rides FP... can group 2 come in the FP line without actual fast passes, only the rider swap ticket?!

I think I really need this spelled out for me, maybe with examples and diagrams.
