Your best bet is to 'bomb' the car - and then leave the windows open for a few days (weather permitting).
After that you will want to vacuum the carpets after sprinkling baking soda on them a few hours before.
Orange Oil can actually break down the chemical composition of tobacco odors - so you may want to try that on the headliner.
Find the fresh air intake and use Odo-ban - it's a commercial strength disinfectant and deodorizer. That should get fresh air circulated through the vents.
As a PP said - if the car has a cabin air filter - be sure to change that.
People just don't realize how horrible smoking is, especially in a confined space like a car. It permeates and stains EVERYTHING.
You might try using ozone. I'm seen the cans for sale at auto parts shops. They are to be used for getting rid of smells. There are also ozone making electric devices that can be plugged into your lighter. I have one, and when used in the past it was great at removing odors.

Interesting. What do apple slices and coffee have to do with deodorizing? I happen to have both, but it seems like a weird combo ! Of course coffe and apples smell better than cigarettes! LOL
