In need of the $10 rebate form for Sonicare Toothbrush

Hi Everyone,

I purchased two of the rechargeable Philips Sonicare toothbrushes that were in the CVS black Friday ad. There is a $10 rebate form that was in the paper. I think it was in the middle part of November in the Smartsaver Coupon pack, but I am not totally sure. If anyone has one that they don't need, I would love to have it! If you could message me I will give you my address and I would be glad to send reimbursement for the postage. Thanks in advance!

Thank you so much for the link, however, that particular rebate is only for people that have the Delta Kansas Dental Insurance. I don't have that. We do have Delta Dental, but we live in Texas. If anyone else has the one from the paper, please let me know. It was open to anyone. Thanks.

Darn I did this but the fine print says the purchase needs to be made AFTER the date of the email. I ordered from Amazon last Thursday. Don't have it yet but the email receipt is dated 11/23. Can't use the other link because I don't have that Delta insurance.

Bummer, is there any way you can chop off the ends of the printed out email and copy it onto a full sized sheet of paper. Sounds kinda shady but worth a shot?

After posting I looked at my email receipt from Amazon closer and noticed that there was no order date listed. SO I printed the email and the only date that shows is the date I printed it at the top! So I am going to send it in and see what happens! It arrived in the mail today so I got the UPS off the box but there was no receipt inside the box so really the only receipt I have is that email. Hopefully they'll honor it! If they do it was a great deal, $34.99 with free shipping and no sales tax, with the rebate it will only be $24.99! Great deal!
