1. It doesn't appear to matter whether it's open or not
2. Nobody really knows the answer there - the lower the better to be honest. If you have multiple biz cards, I'd reduce all but 1 to a very low level and maybe leave 1 at $10k. This will increase your odds of auto-approval while also leaving you enough limit to move some over to get approved.
3. It doesn't seem to matter, you could change the name up if you fee better about it...but many report it's unnecessary
Also - THANK YOU for following up with the detail on the report. That definitely is a positive confirmation for what we expected, CIC, CIU and other biz cards should not affect utilization in any way, and most times it is just confusion when somebody thinks it does (and that's ok!)

I’ve lowered my CL across both business and personal to around 30% of household income before new applications
Before I applied I called Chase to have my SW Biz CL lowered to $1500 (I probably won't ever use that card again). So excited I don't have to call the status line every half hour this time!
ETA: What is the best way to have it expedited? I've only done this once and it seemed like a confusing thing to do with the Chase rep.
