How do you address room requests?

WE are leaving very shortly for DW and I am about to fax BWI and WL with room requests. Who do you address these requests to, and how do you usually word them? We are looking for a lake view at BWI and a woods view room with bunk beds at WL. How would some of you go about wording these requests?
