Epcot Countries Checklist

I was wondering if someone could create a card 4X5 (portrait) of a checklist for the Epcot Countries with Mickey heads. I have seen a ton of drinking checklist, but I am making travel journals for the kids and wanted to add this part in, so they can check off each country we visit.

We found a great, cheap, personalized Hello Around the World Showcase passport online (search the title) - the passport is cute, has the child's name on it, and lists all the countries. Our nephew really enjoyed learning how to say hello in every country.
