Where can I buy these or what's a good site to make your own?

If you book a pkg with Disney they send you one.

I make some with the Mickey head paint chips from Home Depot. I don't put my address on them just my phone # and name. I peel them from the backing and then write the information down on the Mickey head and then I glue two together and then mac tac them (you can laminate) and hole punch through the mac tac between the ears.
