Yes, you can only appear on WWTBAM once in a lifetime. However, they do reserve the right to have people on the show more than once if they so choose. They have done so a few times. There was a tournament of champions during the Primetime series a few years back. There have also been two special zero dollar week series. One was during the primetime series and one during the syndicated series, where they invited back contestants who left with nothing the first time and gave them a second chance. Also, during the Super Millionaire run, each night one of the "3 wise men" was a former big dollar winner from the primetime show. Oh, and you can also be on one millionaire series, too. So, primetime WWTBAM, syndicated WWTBAM, and Super Millionaire all count as one.
Yes, you can win the cruise more than once. It has been done too. I know a person who won twice at DCA when they still had the attraction. I've also read about someone doing it at MGM. I've won once and assisted my DD to another win, so that almost counts as twice. Since I'd have to technically win 2 more times myself to have the most wins (assuming I'd have heard if anyone had won 3 times), I'm hoping that my DW or DS wins instead so that we can go for the family record. I know of a family in California where the husband and wife have each won a cruise. Their daughter was a winner on the kid version of Jeopardy!, so I suspect if the show was still running in DCA, she would have won by now too.
The rule is that once you win, you are ineligible for one full year. You can play for prizes in 30 days, but you cannot win the grand prize. In fact, if you get in the Hot Seat again and successfully answer the 500,000 point question, you are advised to stop. You don't win anymore prizes by correctly answering the million point question and if you miss it you drop back to the 32,000 point level. To my way of thinking, that would cheat the audience out of a good show. Would you want to see someone reach the million point question and then walk away?
