who does personalized menus?

Has anyone had a personalized menu done at a restaurant other than Coral Reef?. How did you arrange it?

The Mrs. and I ate at Victoria & Albert's. Meal, wine tasting and tip was $420.00....got to personalize your menu and the food and service was out of this world

if y0 want to persoanlize the menu (not the food on it), I think you call the resturant and email them info I read it can be done at all table service resturants. Not sure, just what I have read. I think V&A sort of lets yuo personalize the items on it also, once again, info I gather from reading the boards...
