visa waiver form

we are travelling with TCD in October, myself, wife, daughter & her friend.
i seem to remember the forms you have to fill out on the plane were one for each person, and another white one for the whole family.
how would this work with the daughters friend ? can she be added to our form or does she have to fill out another white form just for herself ?

i thought that was the case but just wanted to make sure, knowing how fussy u.s. immigration is.

although actually i think we did speak a couple of years ago, did you say you used to work in the material shop that was at the top of the high street ?

That's right -we used to own it before we retired 15 months ago.
