Permission letter link needed

We are heading to the land of the mouse soon with our nephew without his mom and dad(common-law). I tried to search for the link to the permission letter but can't find it on here. I had it on my old puter but never transferred it over to the new one. Anyone out there have the link?
 
Just an FYI that might help you along the way.....we travelled to Wisconsin via car in June 2010, and took our son's best friend along for the trip. We used one of the form letters from the Government of Canada link above, had the child's parents sign it, and then had it notarized (thinking we'd covered all of our bases). When crossing into the US at Port Huron Michigan, the agent gave us a VERY hard time because the letter didn't specifically give us consent to seek medical attention for the other child (who was 14), even though we had his out-of-country medical coverage information in hand. Because he was a minor, even though we had coverage, apparently we couldn't authorize a doctor to look at him in the event of injury or emergency.

So, it would be my suggestion to add in a second page authorizing you to obtain medical care for your nephew. You don't want the hassle that we had :sad2: . Not a good way to start a vacation.

Pardon me if this sounds dumb, but do I still have to bring a letter if I am travelling with the kids without their dad? We are still married. I'm not sure even after reading the site if it is advisable to get one or if this is specifically for couples that are divorced.
 
I always bring a permission letter when I travel with my kids without my husband. Better safe than sorry.
 
Thanks for your quick response. Does it have to be notorized?

I've never had it notarized, but we haven't flown anywhere in 3 yrs and with all the new regualtions and security measures, it might be a good idea. I'm flying this summer with just my kids and hubby is joining us later, but I'll have a new letter drafted up and get it notarized.
