Need help with an autograph book

I'm looking for somebody to design a Fairy Godmother autograph book page from Minnie and Friends Breakfast at Disneyland. Thank you to any designers that can help.

She is no longer DISigning, but she has several different versions of autograph pages to choose from. Once in her PB, look to the left and make sure you view all the volumes.
