That is nasty. You mind as well buy used underwear there too. I would spend a little bit more money just to know that it is clean.

If you think that buying something from a department store or boutique guarantees that no ones bits have ever touched it you're fooling yourself.
 
Besides, people try on bathing suits at store prior to buying them, yes there is a tiny strip and they tell you to leave your underwear on while trying on bathing suits but do you know how many don't follow that rule? I do follow that rule but many do not and that's why I wash my new bathing suits (and yes underwear) in HOT water prior to wearing them.......
Those little strips are a joke. Half the time they've been ripped off or are falling off. And, while I make sure my kids and I keep our underwear on when trying on a swim suit, I agree with you that there is absolutely no guarantee that others do the same.

I think when it comes to shopping at second hand stores it pays to know your prices. For example, I can sometimes get a better deal at Kohls if an item is on clearance and I have an extra 30% off coupon. I've also scored kids clothing on clearance at JCPenney in the past (before their silly new pricing plan) for $2! So, personally, I wouldn't exclude shopping at a thrift store for sanitary reasons, but I'd make sure to know what items should sell for.
