Where can I find the medical form to fill out to request a refrigerator for my medicine?

You can find it on disneycruise.com under My Online Check-In. But I think to get to that the cruise has to be paid in full before you can access it.
 
You do not have to have your cruise paid off to access the form. After you go to My Online Check In, click on Medical Form and you can download a PDF file.

This is what I did a few months ago for my DH to make sure he had a refridgerater in our stateroom for his meds. He and the doctor filled it out and the doctors office faxed it over to DCL.

Hi everyone, I have insulin dependent diabetes and need to request a refrigerator in my room. I have been to My Online Check In but can't see a category name "Medical Form". Any help would be greatly appreciate.:confused3
 
There is a category for "forms." It has the Medical form, the minor sailing with a non-parent adult, etc. You can also call or e-mail DCL.

If you are on the Dream, all rooms have refrigerators. On the Magic and Wonder you will still need to request one. They will also put a sharps container in your cabin if you request it.
 
I had my TA call DCL and request one just this week, she was told by DCL that I would have to request it in person at Guest Services after boarding, that they were first come, first served. :confused3 I only need it for DS's antibiotics, not for insulin though.

I will find out for sure on Sunday!

Sounds like either the TA or DCL CM doesn't know what they are talking about!

I just went to the DCL site. It is now called "Special Services Form" rather than medical form.
 
I just found it, but when I click on it, it takes me to the disney parks website- it won't open a pdf file. I wanted to check on my daughter's food allergy - to see if they need that ahead of time.
 
We just cruised with two people who have severe food allergies. They do NOT need that form. Just email or call and they'll note it on your reservation. All will be great!

Oops - just read the pp needs a fridge. I think you may indeed need the form for that, but not for a food allergy.

I just went to the DCL site. It is now called "Special Services Form" rather than medical form.

The cast member was correct. They no longer have a fridge waiting for you in the room. A guest had an issue with keeping their meds at the correct temp with the portable fridges. So, to avoid any more issues on the Magic and Wonder, no more fridges in the room. According to the supervisor we spoke with.

If you read the form they are requesting you bring the medication to guest services for storage.

For Guests bringing medication: Will you require a sharps box?
Guests should carry a full supply of all necessary medications and medical supplies with them during travel. Guests should keep medications or medical supplies in original containers with their carry-on bag, and not in luggage being checked. Some staterooms are equipped with a cooler box. Please do not use the cooler to store medications that have specific temperature requirements. If you need to store medicines that require proper refrigeration, please visit Guest Services for 24-hour access to refrigeration.
