Kohls cake pop maker rebate form?

I received my cake pop maker (yippee!) but all that is in the box is my shipping receipt. Is there a separate rebate form I should have received?

you have to print it off, here is the link it is right on top.

I called the 1-800 on the form and was told to send the form, the shipping slip, and the UPC bar code with the model number on top (on the bottom of the box).
