I answered before you tried to delete it. That's probably why it wouldn't delete. I'll leave the answer anyway, in case it's helpful.
What you are asking about is called a Guest Assistance Card, abbreviated as GAC.
They don't issue a GAC before you get to the park, so you can't write to anyone to get one. Instead, you go to Guest Services at any of the parks and ask for a GAC there. In the past, they did take the information by phone, letter or email, but the decision on whether of not you would get a card was still up to the CMs in Guest Services. The person the GAC is for needs to be with you at Guest Services and you will be asked what dorts of assistance you need or what sorts of problems you will have with waiting in line. You don't need a doctor's letter, but a lot of people feel better having one. If you haven't gotten one already from your doctor, ask for a letter that specifies what your child can't do or needs (like a place to wait out of the sun, or in a cool place). A letter that just lists medical diagnosis and medical terms won't help since the CMs are lay people, without medical training.

Thanks for the answer. My family might be a real mess with 2 of us sporting those plus me being in an ECV. I already have had reconstructive surgery on both feet and will never again be able to walk the parks. I'm on narcotics for intense joint pain that may result in surgeries on both hips before our trip in November. My 10 year old has been sick for a year and getting worse, and still we have no diagnosis. All the docs can agree on is that it's very painful, and many times he is in no shape to stand, much less in a line. I'm hoping we'll have a diagnosis and relief for him before this trip, because I want him and his younger brother and sister to be able to enjoy this trip.

That also answers the great mystery of why I couldn't delete.

My experience has always been that the CMs at the parks and resorts go out of their way to help us and try to make our vacations special. The GAC will help facilitate that in a way, just by being an indicator that your family needs special assistance. It isn't just about lines and rides. Any time you are feeling overwhelmed or in need of assistance, you should ask. Also, know that the First Aid stations are wonderful places if you or your child get overwhelmed.
