How do you thank CMs?

Someone mentioned something about forms you can fill out at City Hall to thank CMs. Does anyone know about that? Will they let you stop there at the beginning of your day, pick up one or two forms, then fill them out if something happens and submit the forms at the end of the day? And does anyone know if this is possible at the other parks? Can you stop at Guest Relations as you enter the park, pick up a form or two, fill it out and return it later? If so, I was thinking of doing this on my next solo trip-picking up a form when I enter a park, then while I'm sitting at a resturaunt, I can think of who to thank and why.
 
I make a short voice recording on my cell with the info then write them up later that night or in the morning.

Just remember, cast members have rotations. So if Chris at Space Mountain was "super" - you'll want to make sure you know it was tall, brownhaired Chris at Space Mountain between 3:00-3:15 at the entrance of Space Mountain that did _____ to make you feel special. :-p

More specific you are, the easier it is for Guest Relations to catch up with that person's manager. I know I once recieved a "Guest Service Fanatic" card that took them 3-4 days to finally catch up with me.
 
Is there any way to send a thank you after you've left the parks? I had thank you cards printed up, but when we went to Haunted Mansion on the first day of our vacation, we were so busy handling the transfer of my DD's fiancee from his wheelchair to the doom buggy that I completely forgot to give one to the fabulous young woman who helped us out. (She also made arrangements for us to go in the stretching room before our ride. This was especially memorable because this was my future son-in-law's first trip to Disney.) I don't remember her name, but I might be able to figure out the time that we were there. She not only greeted us at the handicapped entrance, we saw her later at the general loading area.
 

I send letters to Guest Relations complimenting special CMs. In fact, I got a phone call from Disney yesterday morning thanking me for a recent letter. The lady said it was much appreciated and they had shared my letter with the CM...so, those letters do get read.

I also always ask the CM if their supervisor is available to speak with. I like to make person to person contact, if possible, to compliment the CM. I follow up with a letter to Guest Relations if I am able to speak with a supervisor but I send a letter regardless. Sometimes, I have been given the email addresses of the CMs boss/bosses and I send a note to them personally.
 
My 6 year old daughter gets some cast members to autograph her books. She asked a really nice bus driver who dropped us off at the Poly for our 7 am breakfast. She also got Belle's assistant at Belle's Storytelling. Both were pretty shocked and I think it made there day!
