If a cruise ship has a 4:00pm depart time do they leave exactly at that time?

Yes. Normally.

If it says - depart, that is the depart.
If it says - all onboard, then no, it's a little later.

In Europe the Magic has - all onboard, 15 minutes before departing. That is normally, not the case say in the Caribbean or Bahamas.

On Embarkation day, at the start if the cruise, they have - all on board before the mandatory drill. They leave later.
 
Last year we were late departing by about 20 or 30 minutes waiting on a family to arrive. From what I heard they called Disney and they waited on them. Always wondered if this was true or not.

It could be. If they were booked by DCL on their flights, and it's possible to do so, DCL has waited for late arriving passengers.
