This will be my first trip with little one. We are staying at the Poly so that if breaks are needed, it's easier to get to and from the parks. I usually make fastpasses for late morning and afternoon if I can. I do it that way because it's busier and when those are used, I still have time to book more, if they are available. My daughter will be 15 months when we go so I'm thinking that afternoon won't be a good time to book fastpasses since this might be nap time. Any input as to times that I should book my fastpasses for?

We like to make FP+ for 10-11, 11-12, 12-1. That way, we benefit from the lower crowds earlier in the day before taking advantage of FP+. I think the same system could work with a 15-month-old as you'll be able to use your third right at noon and then head out for an afternoon break. It's so much nicer now that you're able to book your 4th FP+, etc. on your phone as you'll be able to do that straightaway for the time you think you'll be ready to head back to the park.
 
If you are planning to utilize Rider Swap, I'd book the times the PP stated for the height-restricted rides and then Ride using the Swap Passes after you return in the evening.
 

If you're doing midday break, ideally I'd do something like 9:30, 10:30 and 11:30. Spend 9-10 doing standby on a few rides in fantasyland while everyone else tries to do mine train. Then at 10 do your first two FP rides. After your 3rd, make your next FP for later. You're going to find a lot of 4th FP availability for many of the rides you'll do with the 15 mo old.
 
Ours have been pretty young in our travels and we kind of like to be wrapped up/done by well before 1 and heading back to the resort for naps. Doing FPs 9-10, 10-11, 11-12 or 9:30-10:30, 10:30-11:30, 11:30-12:30 has worked pretty well for us.
