Do you have to be in the park before the EMH actually starts?

No..........The wristband disbtibution starts 2 hours before the evening EMH but you don't have to get it at that time. You can arrive after EMH starts and as long as you are a Disney resort guest you will be admitted and given the wristbands.
I did hear a CM back in June say that they sometimes run out of wristbands. I doubt that this happens much. However, if they did run out I don't think you would want to be at that EMH anyway since it would then be packed.

Can you get the wristbands at both entrances to Epcot? We're planning to eat at Beaches & Cream and then use the International Gateway to enter Epcot for EMH on 10/4. Will that work? Thanks.

We did not use the International Gateway but I would imagine they are there. There were several spots inside the parks to get the bands as well. All we had to do was ask a CM where we could get them and we were told where we needed to be.
