Disney with a blind 6/120 child

Hi all. We are getting ready for our 1st ever Disney trip. Our younger DD (age 7) is legally blind as her vision is 20/400 (6/120) I am going to get her an assisntance card ("GAC")once we get to DSLP. I have been to old to bring a note from our doctor who is a pedi-opthamologist. Now am wondering how should the letter be worded. I mean should it just descibe her condition (Albinism, low vision & photosensitive) or should the letter also actually SAY that she needs: "front row/front of show & alternative enterances for rides."

Hi - a straight forward doctor's note describing her condition should be fine. We still use an old benefit book for my Db who is autistic. The book is just an old disability living allowance book and doesn't really outline his condition or what help he needs.
