Welcome to My Online Check-In, your one-stop place to manage important documents for your magical cruise vacation. Here you'll find:
* A Cruise Summary
* A list of all Guests on your reservation
* Easy access to forms to fill out online prior to embarkation
By completing these forms online, you save time and make boarding your Disney Cruise Line® ship a breeze.
Just follow these six steps:
Step 1
Fill out the Guest Information Form for each Guest.
Step 2
Fill out the Payment Authorization Form for each Guest. The form must be signed by the credit card holder or, if you are setting up a cash account, by the primary person responsible for all onboard charges.
Step 3
Read the Disney Cruise Line Cruise Contract by clicking the "Fill Out Form" button next to "Cruise Contract" below.
Step 4
Select "Continue" to agree to the Contract and print out the Online Check-In Signature Forms, which includes the Payment Authorization Summary. All adult Guests in your travel party must sign both forms.
Step 5 (optional)
Fill out these forms, if they are appropriate to your vacation needs:
* Flight Information Fill out this form if you are flying and did not book flights through Disney Cruise Line®.
* Flight Modification Fill out this form if you booked your flights through Disney Cruise Line® but would like to make a change.
* Medical Information Fill out this form for wheelchair access or other medical needs.
* Minors Authorization Fill out this form if you are traveling with a child of whom you are not the parent or legal guardian. The Minor Authorization form must be notarized.
Step 6
Bring the following items with you to the port terminal**:
* Signed Online Check-in Signature Form
* Signed Payment Authorization Form
* Signed and completed optional forms from Step Five
* Valid proof of citizenship for all members of your party including children
* Valid government issued photo identification for all adults in your party
All forms must be submitted at least 48 hours prior to the start of the vacation, except:
* the optional Medical Information Form, which must be received by Disney Cruise Line® at least 14 days prior to embarkation, and
* the optional Flight Modification Form, which must be submitted online at least 60 days prior to embarkation
Please note that you will be able to fill out any incomplete forms at the port terminal**.
