Which restaurants offer a personalized menu?

I know Coral Reef does. But how many others are there and how do you go about getting one? Should I call the restaurant directly?

We got them at Yachtasman Steakhouse, without asking. We just noted it was our anniversary when we made out ADR and they were waiting for us at the table. It was a nice touch!
