I would recommend Princess Storybook in Norway. But no, the princesses, don't bring out any cake. You may be able to arrange a special cake to be delivered by your server. There are links on Restaurant thread regarding ordering special cakes.
When we've been at PSB for DD's birthday, they've done different things. Most times they bring a birthday card signed by the staff. Once, at lunch (before it was all princesses) she got to wear a cape, and hold a sceptor, during her meal, plus they gave her a Magical Moments Certificate to celebrate her special day.
Make sure you get your DD a birthday button, and she can wear it for more than just her special day. When we've been there on, or near, DD's birthday, she'll usually wear it for a few days since we are celebrating her birthday during the whole trip. However, when she was younger she was quick to point out that it wasn't really her birthday! I would just tell them that we were "celebrating" her birthday that day. If she wears the button, she'll get some special attention.
I recommend getting (one) button at Epcot Guest Relations. Most of the time (if they aren't too busy) when you ask for a button, they will ask your DD's name. After you tell them they will say they had a call for her earlier. Then they make a call, and Goofy comes on the line to talk to your DD. It's great! This has happened a few times to my DD and she thinks Goofy is very funny. We had this happen twice at Epcot Guest Relations, once at Mouse Gear (store) in Epcot (FW), and once in lobby of Brown Derby. It's definitely a Magic Moment
