Oh, good. I had submitted his name for another show (but I can't remember what it was) and it never occurred to me to ask him or even tell him, for that matter! If someone will PM me the info, I will fill out the form, too. I think you guys are great for doing this for him and his family.....I just want this frustration to come to an end for them!
