good morning all please can someone tell me please the white and blue customs forms you have too fill in , i have had so many diffrent advise on this i'm confused is it one per family name,what if you have same name but diffrent address'es?

You need to complete one customs declaration form per family name.

Unless of course you mean the visa waiver form as per the post title.
In which case a completed I-94W "green waiver form" needs to be completed for everyone.

In summary, one visa waiver form per person, one customs form per family name.
Addresses irrelevant in all cases.

We did as advised above re customs forms on our trip last year as we have different surnames. However the guy at immigration said we only needed to fill one in as we live at the same address!

The white customs for is one per family name. It doesn't have your home address on it so if all your group have the same surname you only need to fill in one

Looking at the advice from US customs it does say
"Each individual arriving into the United States must complete the CBP Declaration Form 6059B. If you are traveling with other immediate family members, complete one form per family unit"

I always took it to mean those with the same surname as the form suggests but it could be interpreted as any family members that you are making the declaration on behalf of.

It's interesting isn't it because, with the way they're changing things, my DF and I will be considered a family unit officially in this country soon, as we own a property together and are engaged. Mind you, we consider ourselves a family unit already - me, him and the cat lol!
