Triple (or Quad) Mountain Card

A few years ago, I found a card here that had all the mountains listed and you could check them off. I have searched and search for the original but can not find. Can anyone point me in the right direction?

here is one I made...maybe if you don't find the other one this might work....just check mark the Mickey Hand when completed.
