I think the Japan restaurant in Epcot and the California Grill

Plus, I heard Kona has a sushi bar at the Poly, but haven't seen it myself.

From what I have read and pics I have seen Captain Cook's at The Poly has sushi from Kona on the menu.

Kona does have a sushi bar. They take TS credits, not CS, although you can carry out the sushi. You can also order it if you are eating at Kona.
