"You may cancel your MLPA Membership by sending an email to
memberservices@medlifeplus.com, faxing a written request to cancel to (678) 819-0529 or mailing your written cancellation request by US Mail to: Member Services, P. O. Box 674134, Marietta, GA 30006. (Please note that cancellation of your MLPA Membership does not make you eligible for a refund of membership fees incurred before your cancellation request unless you cancel within thirty (30) days of enrolling and request via email or in writing a refund of your $1 enrollment fee) Your MLPA Membership will be cancelled and an email will be sent confirming your cancellation. This email will include your Cancellation Confirmation Number. If you do not receive your cancellation confirmation email and you have provided us with an updated valid email address, your request to cancel your membership has not been received. Please call 1-866-463-6237 from 9:00 a.m. to 5:00 p.m. Monday through Friday Eastern Time, to confirm your cancellation.

http://www.thejournalmanual.com, ".should Member choose to discontinue membership in a Plan, Program or Benefit(s), Member must notify Member Services via email or writing at least five (5) days prior to the Member's next billing date to avoid being charged for the next payment due. Refunds for enrollment and/or membership fee(s) are not automatic. All refund requests must be submitted to Member Services in writing for review and approval. Member Services shall notify Member via email within five (5) business days of its decision and, if approved, issue the refund(s) within two (2) days."
If you request to cancel your membership
at least five (5) days before your next billing date,

Your membership will automatically expire at the
end of the current billing period."
