Size of design on T-shirts

Hi:
You can tell I have never done T-shirts before. This is probably a really dumb question but how do you size the designs for different size T-shirts. For example, the one for a six year old would be smaller than the one for the adult male x-large. Is there a way that I do the t-shirts so proportionally they look right? Any help would be wonderful.
 
I go to the printer preference and where is says advanced click on that and then a box will drop down, then you will see the "paper size" click that and it will give you some choices. I use the 4x6 for smaller shirts and 5x7 for the next size. But I can say, that some of the designs do look better big. I too had to make 3 different sizes and a few of them, I preferred to use the same size...run it on plain paper and see what you like. Better to use ink on plain paper than to ruin an iron on. Thank you so much! I will definitely do the plain paper before using the transfer paper. I won't have known where to begin. Really appreciate the help.
