We are heading to the World in September, me, my DH, and my 3 year old. She is very tiny, as she is a former micro preemie; she currently weighs 26 lb, just for a reference.

I am wondering what rides, specifically in MK, that all three of us can ride, and which ones we will need to separate on. I'm thinking rides like Buzz Lightyear, Peter Pan, Winnie the Pooh, Dumbo - we have been before but our last trip with her she was 15 months and we didn't ride a lot. I do recall all three of us riding in one elephant on Dumbo.

Also, in HS, Toy Story Mania - does it hold three or just two?

All the ones you've listed we've ridden 3 on at some time or another except, Buzz and TSM. For those rides, three can definitely fit in the seat, but there are only two shooters so the third person won't be able to play.

Aladdin's carpets: same carpet, different rows (2 rows per carpet)
Small World, Teacups, POTC: bench seating

We did three together on the rides you mentioned too.

The new Little Mermaid (Under the Sea?) ride can fit 3/shell. Haunted Mansion will fit three cozily also.

We've done three on TSM and Buzz. It does mean two blasters, but it made it easier for the smaller child to reach by sitting on a lap.

We're a family of 5 so we split up 3 and 2 quite a bit in the past - especially when all of our kids were under 7 years old. (They have to be 7 not to ride with a parent). The only tricky ride I remember is Speedway and that's only because all the kids wanted a turn driving.

Buzz they can fit 3, unlike Toy Story Mania one person can spin and two can shoot!
