Where do you go to get a passport form? Would a post office have them? I got mine years ago & don't remember where I went to get the form. Also, my passport expired about 5 ~ 6 years ago....would that still be considered a 'renewal'?

In my city, you go to the main post office.
 
I got mine from the post office that delt with Passports, not all do. So I'm not sure if all of the Post Offices will have them or not...just call your local Post Office.

I believe that if you've already held a passport then it is considered a renewal and therefor the full charge of a new passport won't apply.

The passport forms are in Adobe format. If you don't have Adobe on your computer, you will need to install it. It is widely available for free on the internet.
 
try http://www.travel.state.gov/ and follow the links for get or renew a passport. It took me less than a minute to find your answer (and I have slow dial-up).

Already have a passport that is not damaged; and
Received it within the past 15 years; and
Were at least age 16 when it was issued; and
Still have the same name as in passport (or you can legally document your name change).

It's important to call first. Our PO only processes applications on certain days of the week and only during certain hours the PO is open.
