Need Help With Documents

I'm just filling out the documents and for the Departure Information Card, we are renting a car from AVIS at the Radisson Hotel in Cape Canaveral. I don't see a spot for filling this out if we are not leaving a car at the Port during the cruise but just want to make sure....
Also - for the Bahama's Immigration Form, we will only be visiting Castaway Key. Is that the "Other Island" specified on the form?

You don't need to fill out anything regarding your rental car if you are not parking at the port.

And yes, it is "other island" for the Bahamas Immigration Form if you are on the 7-night cruise.
