medical permission/notarized

I am writing a letter to be notarized giving my bil and his fiancee permission to seek medical attention for my children if needed. I know I have to sign it and have it notarized, but do they have to sign it as well? thanks

Each time I have had this done I was the only one to sign it.
 
The legal form I have on my computer only asks for the parent signature to be notorized.
 
Whenever I take kids with us I only have their parents sign and get it notarized.
 

Is it possible to call the medical clinic where they might be treated if needed and see what they require? I know here a parents signature is enough but other clinics might require notarization. Don't forget to add any dental treatment to your letter. I also include all contact information, cell phone, hotel phone/fax, etc. and all medical plan numbers, etc so it is all in one place if needed.
 
Wow, I never thought about having it notorized. Makes sense. We sent DD to Florida to my niece's for a few weeks last summer...I just wrote a letter and signed it.

But I would imagine it would just be a parent's signature required.
 
It is a good idea to have it notarized just so you have the legal authority to have the kids treated. In a true emergency, the ER will treat them regardless of your paperwork. They like to have the parents address & phone number. It's also a good idea to copy your insurance cards, front & back, so that insurance can be filed promptly.

I had to take a teenager to the ED this summer while we were on tour. She broke her nose and was pouring blood. The ED staff never batted an eye--got her in, Xrayed, etc and we never did reach the parents until almost 8 hrs later. :earseek: Had she had a more minor illness they may have waited until the parents could be contacted. Blood gets their attention. pirate:

No, they do not have to sign it. I'm the manager for my sons soccer team. We have these forms filled out by every parent and notarized. We don't even put a name in the spot where you state who can make the medical decisions.

If another parent or coach has to take a child to the hospital, (either by car or ambulance), we would just fill in the name on the way.
 
I would check with your pediatrician. I used to write a letter every time that my 4 kids stayed with the grands (in state). The pediatrician told me that in our state, whoever brings the child in for treatment is able to receive such with no parental permission necessary.
