Yes. The ADR is so the restaraunt knows what sort of table you will need. An infant does take room at the table even if not eating, or only sharing from your plates.

Yep they want to know the names and ages of everyone on your reservation. I will have a one month old and had to include the baby on my reservations for Chef Mickey's!
 

Your infant (regardless of age) will also count toward party size. If you have six or more in your party, including the infant, that will trigger the 18% automatic gratuity.

Like everybody has already said. A party of 4 adults and an infant, no matter how small, will not fit at a "4-Top" Table. There will not be any room for the highchair or the infant carrier. You would need (and want) a "5-Top" Table. Doesn't matter who's eating or not.

Our first two trips with our youngest, several Disney restaurants tried to put us as at 4-tops. It was slightly irritating. We made due at some because the table was large enough (WCC was one) but at others it just didn't work and I requested a move (very cramped at CP, they were very annoyed about it too).

Not that this changes anything: infants should be noted on ADRs.
 
One CM told me this was necessary to ensure fire code occupancy rates were followed
