How does the dining plan work?

How much extra is it to add the dining plan to a package? How do the credits work as far as how many you get, how you know how many it takes to eat at a certain place, is it per person or the whole family etc....
I have not been to Disney since 1992 and this is all new to me.....Can't wait to get there with my hubby and our two kids.
