We have reported posters to their ISP, I can't remember every having to turn to "real" law enforcement although I do recall contacting the police about someone who posted what was basically a suicidal note.

We can't always tell but there is always a way. Ever computer on the internet has a unique address called an IP address. Sometimes it's in a form that makes it very obvious where you live other times only your ISP can tell but they will give that info to the police under the right circumstances
