FIRST POST in this thread is an up to date summary of how to request DAS and how it works.
Only the first post is important to read. Most of the other posts are from right before and right after DAS changes were made in October of 2021. Some are more current experiences with requesting, but didn’t change anything.
To answer your specific questions quickly:
Do we call?
No. You can either register for DAS using a link on the WDW website or in person at one of the 4 theme parks.

Done by chat? Not available right now?
It starts out as a text chat where you will wait until a Cast Member (CM) is ready to talk to you. The CM sends out a request for you to connect to a video chat where you will be asked a few questions about your concerns with waiting in line. Be ready to discuss those concerns, not just list your disability/conditions.
If approved, the CM will take your picture and register you for DAS. Live chat is available from 7:00 AM – 10:00 PM Eastern Time.
If it says ‘not available’ all the chat slots may be temporarily full and they are not taking any more at this time. If you are outside the US and Canada you may also get that message since the chat is not officially available in Europe.
Can it be done at the resort?
No, not at your resort or at Disney Springs.
Registration can only be done by video chat or in person at one of the 4 theme parks.
