How could DCL "bump" us from our HA room without checking with us first since we have our room assignments?

If DCL has your reservation noted that you need the accomodations of HA room, you should be ok. We filled out the special needs form, just to be on safe side. Didn't need a dr. signature.
They can't ask your diagnosis, but they can ask what accomodations you might need.
