Grandparents taking 3 year old grandaughter! Help!

Hi, I need help with a letter stating that grandparents have the permission to travel with and make medical decisions regarding our 3 year old granddaughter ! Would any of you have one or any ideas of what should be included? I am already planning on taking a copy of her birth certificate, but I am drawing a blank on what this letter should say.. I am also plannign on having it notarized. What have you all used.. when taking friends etc? thanks in advance!
 
We travel with my great-niece, and I did a google search for Medical Permission Minor and got several samples that I retyped. My niece filled it out and got it notarized and we travel with that and her insurance card. Knock on wood, we haven't needed it yet.

Make sure you also have their health insurance membership card or membership number with the company's 800 phone number in case you have to get pre-approved for medical care.
 
If you are traveling any other way but driving you may also need a notarized "permission slip" from BOTH parents giving you permission to take her on whatever transportation you are taking. Airlines may ask for something like that if the names do not match. You can probably include this in the medical permission slip, if you want. Might be a little easier to have them separate.

Have fun. Our son was 3 for our trip last November. He loves it more and more each year we go. The parades weren't too big of a hit but the character meals were a highlight. Also saved us having to stand in line to get autographs and pictures.
 

Thanks! I appreciate the help! We have made several.. ( or more) trips to WDW with family but none with just children and our selves! I am starting to panic!... in a good way!
 
Should be tons of fun. Remember it doesn't matter what you do as your grandchild will just be pleased to be with you ... and at DisneyWorld ... there can't be a better combination. Good Luck!
