My grandparents farmhouse was built before indoor bathrooms were a thing. Apparently when my mom went to the house for the first time, they had a similar setup, with a toilet on the stairway. It was use that or the outhouse. At some point after that, they had a bathroom built that you accessed via a breezeway off the kitchen (which is the only bathroom I remember there).
One thing worse that that is when they put a window with foggy glass in a shower!!! We went on a river cruise and that was the configuration... fine on a sunny day, but turn the light in the bathroom on, and anyone outside (walking by on the docks?) had a nice peep show!!
My MIL came here from Scotland. She was just stunned when we were in several places where you can see another one from the one you are in.
