Official dates for 2016 have not been announced yet. In 2015, the dates were Sept. 25 through Nov. 16.

if you have to guess, it may be September 22 to November 14. But that's not official. Disney hasn't released the dates yet.

When were the menus released last year?

I've read the dates are September 23 - November 14, 2016. Apparently the dates were on the back of the Holiday Passport 2015 for Candlelight Processional (just read this, didn't actually see the passport).

When were the menus released last year?

The menus are released pretty late 1-2 months before hand. However, usually 90% stays the same with a couple of new booths or a few new offerings. Here's our reviews from 2015.

They were on the back of the holiday passport for 2015 too. And then Disney changed them by adding an extra week in November.
