Flying from Toronto with Grandchildren

My MiL will be flying to Orlando from FL with 3 of her grandchildren. Normally I travel with her and help her with stuff but we tend to fly out of Buffalo. I'm preparing a document for her to help her through the steps.

Does anyone know how many customs forms she will be required to fill out in Toronto? Between herself and the 3 children they live at 3 different addresses. Does she fill out one form for each home address or can she put the children (all minors) on her customs form?

PS - She will have the correct paperwork signed by each of the parents granting her permission to take the children to Disney.
 
Just a note the letters of permission should be notarized. Even when a couple is still together if only one parent is traveling solo they should carry notarized letters. Many people don't think of that.

Here's a link to the US customs form. It mentions 1 form per family, but doesn't specify that they have to live at the same address: US customs form

The Canadian one - which they will have to fill out on the way back, DOES specify that they all must be living at the same address, which means she will have to fill out 3 of these. Canada customs form
