If my right hand is in a plaster cast so that I can't stick my finger in the scanner at the gate with a ticket I had previously used with my right hand, how do they handle it? Do they need to scan my whole finger or can they scan just my fingertip if I can maneuver it into the scanner?

They'll just call over an iPad CM and they'll reset your biometric. Use your free hand.

My finger wouldn't scan on the second day after we bought our annual passes. I have no idea why not but as a PP said, a Cast Member was called over with a pad and he just reset the entire thing very easily. So, just go up to the scanner and mention to the Cast Member there that you need to have it reset and it'll be taken care of right there so no need to wait in a line at Guest Services.

Yea, I've seen people us either hand, thumbs, whatever. You wont have much of an issue.

The issue isn't which finger you choose to use.
The issue is which finger is currently associated to that ticket from a prior entry. The OP's right hand was used in the past and is associated to that ticket. The bio-metrics need to be reset to the new finger.
Totally different issue.

and just as a side note they may take your picture on the ipad when they're resetting it. DH forgot which hand he used and ended up getting his reset they took his picture on the ipad and I think checked his ID

Some issues that people have is that they press their fingers too hard on the scanner and it doesn't read it correctly. I've heard a few CM's tell guests to not press down hard. I've always just lightly put my finger down and have never had an issue.

I've been to Disney several times, but on one particular trip, I had horrible issues with my biometric. After the 2nd day of trouble, they had a cast member with an iPad come over and reset my biometric. It was easy to do and the trip went much more smoothly after that. Good luck!
