If you go to the UPS or FEDEX websites, they can give you a quote on price and length of time.
UPs, Fed Ex, DHL, any kind of courier service will deliver directly to the hotel. My suggestion would be to get the package ready and send it out about 7 days prior to your arrival (you can send it further out if you like they will hold it). If you give yourself enough time you can ship it ground service which is the cheapest.
As for who to address it to you would want to address it like this:
The name of the person who is the LEAD name on the reservation (may not be you, but it is easier to find that way)
C/O the hotel you are staying at/ Front Desk
The address of the hotel
And put HOLD for guest check in and the reservation number if you're the diligent type, it's not required though.
So it would look something like this

For packages arriving to the resort, the guest name and arrival date are essential
On the labels first line, include your name preceded by the word Guest and date of arrival ( i.e. Guest Minnie Mouse 08/27/11)
Your package will be held up to 2 weeks prior to your arrival
Pick up/ship your packages from the Concierge Desk
I made sure I had her arrival date on the package. I took the box into a UPS store with the information and they did the rest.
