Disneyland & Big Thunder Mountain Death ??

Hi.
I know everyone is a WDW fan (like myself) but I figured someone would have some insight on the death of the passenger riding Big Thunder Mountain in Disneyland (occurred maybe like a month ago?). I've looked online for the full story b/c I never caught more than a clip-it on the news. What exactly happened? Was it a freak accident? My heart goes out to the gentleman's family.
 
Check out the Disney Rumors and News Forum - there were a couple of threads going about this a while ago.

A report has been released - sounds like a wheel from under the decorative locomotive was loose, castmembers heard an odd sound, and were going to take the train out of use after it went around. It never made it around. Apparently, the wheel came off, or jammed, causing the locomotive to derail/stop or something like that. There was a very detailed report available on the 'net a few weeks ago, I'll try to find it for you.

I have also been trying to find more inf on cause of accident...I have been checking original sites that reported but last report was for funeral services.
