When you dine at Club 33 you also get park hoppers for free, that is included in the price. There is also a 10 year wait list to be accepted. The $10,000 is a one time fee, then you are required to pay the yearly fees.
As for the dress code, most people who dine at Club 33 wear nicer clothes but there are also people who dine in shorts and tank tops, it is a theme park after all.
